Allow blog authors to manually remove email subscribers.

Background

We've fulfilled this request manually in the past. Also mentioned on the forum: https://discuss.write.as/t/new-in-labs-email-subscriptions/480/40

Implementation

Add a "remove" button to the Subscribers list. Clicking it should confirm that the user wants to remove that email address. Confirming should immediately delete it.
